what is supposed to happen? Is it a penalty and the offense keeps the ball?

There are 2 things at play here (nope there aren't)

1) At the point of the fumble, the defender being in bound or out of bounds when he forces the fumble has no effect on the play. While the ball is in possession of the offensive player then it doesn't matter if the defender is out of bounds. And since the defender didn't touch the ball, while out of bounds, after the fumble the ball was live until it bounced beyond the goal line and out of bounds. Called 100% correctly on that front.

now then...

2) In truth a flag could have been thrown for the defender running out of bounds on his own and continuing the play. However that typically is only ever called if a player runs well out of bounds (like into the player area). But, by rule, it *could've* been called.

However, the part that is being complained about was called 100% correctly.

There’s not a scenario that a defender could have been flagged afaik. Only members of the kicking team and eligible offensive receivers are prohibited from going out of bounds. No rule exists for defenders.

Well, after further review, you are correct

I looked into it yesterday and misinterpretted a rule that applies only to free kick downs.

I think it will simply be a loose ball anyone can recover if it stays in bounds. If it goes out of bounds the offense keeps it because they had last possession.

At this irrelevant and immaterial.
